School Vape Consolidation Form

This form is to notify the Department of Ecology that a school district will be consolidating vape waste using the Interim Policy for Vape Waste in K-12 Schools (Step 3 of the Interim Policy). Consolidating schools must have an EPA/State Identification Number before filling out this form.

Note: Only one form per school district needs to be filled out, but if you add more generating schools at a later date, submit a new form with those additional schools.
