Request for Hazardous Waste Generation Fee Waiver or Exemption

Use this form to apply for:

A one year waiver if you did not generate any amount of hazardous waste in the year billed.
An exemption if you never generate an amount of hazardous waste.

Your opportunity to apply for a waiver or exemption is lost after an invoice is referred to collections.

Instructions

Submit your request by the due date on your invoice.

To fill out the form, you must:

Fill in all fields.
Provide a correct email address.
Provide your eleven-digit invoice number.
Answer the questions that apply to your business type. If your business type is not listed please use the "All Others" section.

After we receive your request, we will respond within a few business days by email. We may request more information in order to make a determination.

Lack of information will result in a denial of your request for relief.
